Hi Blurtians, how you doing?. Recently most dental assessment exams usually ask about the extraction of mandibular molar or premolar in order to check out the understanding of the students as regards dental extractions in general.

So to make it short, I would be explaining about the presentation of mandibular molar or premolar close extraction in this post.

The necessary steps to be followed will be stated below accordingly. So stay tuned.

1- Introduction to the examination with an assumption of the patient's even if there is no patient around and also introduction to the patient with consent seeking.

Good morning sir/ma, my name is dental officer wxyz. I want to demonstrate close/ simple dental extraction of mandibular molar/premolar.

I will greet the patient humbly, explain the procedure with safety reassurance and then take consent

2- Listing the armamentarium needed to the patient accordingly

E.g; I will need my face mask, gloves, local anaesthesia cartridge, dental needle and dental syringe, mandibular molar/premolar extraction forceps, dental elevator( if required) , gauze and cotton wool rolls.

3- Assemble the local anaesthesia and parts together and administer it to the appropriate anatomic location and structure
E.g
I will assemble the local anaesthesia component parts together and administer it and ensure that the local anaesthesia has been achieved.

4- Engage the indicated tooth with the appropriate instrument
E.g by saying
I will first engage the tooth with the forceps and do apical(downward) movement or thrust; to allow for proper grip of the tooth by the forceps.

Next, I will start buccal movement and proceed with buccolingual movements, and some additional rotational and figure of 8 movements if there adjacent spaces beside the indicated tooth.
If the tooth is not permitting any movements in its socket, a dental elevator could be employed so as to assist in the tooth luxation.

Once the tooth is out of the socket, I will first check to be sure that all parts of the tooth are complete and intact together with all the roots are completely out with remnants in the socket.
Leaving anything behind could lead to complications such as dry socket, necrotic bone, halitosis and so on.

Penultimately, I will compress the socket with a piece of cotton wool or gauze, then place another piece on the socket and ask the patient to bite for like 30mins to a hour in order to stop bleeding and avoid spitting out saliva to prevent dislodging blood clotting from the socket.

Lastly, I will give post operative instructions with prescribe appropriate medications.
