LEARN TO DOCUMENT THINGS

Good afternoon family. Trust we are good.

While I was still in the University, one of my lecturer handling Pharmaceutics would always tell us that in the pharmaceutical industry whatever you don't document ,you didn't do it. Meaning, unless you document a thing, it can't be taken into account. The essence of documentation is for future reference and to limit the chances of making mistakes or error.

Lack of documentation has cost so much to so many. It has led to great hazards and even lost of lives. Take for instance being confused on what medication to administer to a patient because the treatment plan wasn't documented; of course this would definitely lead to medication error and it consequences could be mild or fatal depending on the nature of the medication that was administer.

Being a health personnel, I have seen alot of errors that has happened during practice ,most of which, was as a result of lack of documentation. For those in the industry, you would agree with me that documentation is a very vital aspect of industrial activities,without which, no meaningful progress can be achieve.

MAKE DOCUMENTATION AN HABIT AND REDUCE THE CHANCES OF ERRORS.
