Mianwali is an ancient city located in the east of the river Indus. The city was founded in 1868 by a spiritual saint, Syed Mian Ali, who had migrated from Baghdad, Iraq, and the city was named Mianwali after his name. After his death, his son Hazrat Syed Sultan Muhammad Zakaria took over his mace. Hazrat Syed Sultan is very famous among the tribes there, especially the Pashtun tribes. The Pashtuns were of great importance because of the settlement of disputes among the tribes through reconciliation. The descendants of Syed Mian Ali are still respected in Mianwali.
Before 1911, Mianwali was a part of Bannu district of Khyber Pakhtunkhwa. Later it was separated from Bannu in 1901 and included in Punjab. Before entering the province of Khyber Pakhtunkhwa, Mianwali is the last district of Punjab in terms of border, its borders are with the districts of Kohat, Karak, Laki Marwat and Dera Ismail Khan of the province of Khyber Pakhtunkhwa, while it meets the districts of Chakwal, Attock, Bhakkar and Khushab of Punjab.

Niazi is also a Pashtun tribe that evolved from the Yousafzai and Orakzai Pashtun tribes of Afghanistan and Pakistan. Three languages are mostly spoken in Mianwali – Saraiki, Pashto and Punjabi. The most popular language spoken in this city is Saraiki. However, according to the 1998 census the most spoken language is Punjabi followed by Saraiki and then Pashto respectively.
Mianwali is also a very important city politically. Former Prime Minister of Pakistan and Chairman Tehreek-e-Insaf also belong to this city. He was elected Member of National Assembly from National Assembly seat 95 in General Election 2018 and later elected as Prime Minister of Pakistan. Pakistan Tehreek-e-Insaf is the most popular political party among the people here. Former Governor of West Punjab Late Amir Muhammad Khan belongs to Jawawan tribe also belonged to Mianwali and was famous as Nawab of Kalabagh.
