It is very important to inspect and observe every patient brought into dental clinic clearly and effectively as sometimes patients especially older ones may have some sort of diseases or ailments which would required some modifications and manipulations of the normal routine treatment plan in order for them to be effectively treated and as such this kind of patients are referred to as medically compromised patient
There are a lot of diseases in the dental literature that are known to have major impact on the periodontium and general oral health and therefore needed to be attended to first before attending to the dental treatment which makes patients with such diseases to have treatment procedures entirely different from dental treatment of a normal healthy patient
These diseases are mostly systemic disorders which have manifestations on the oral cavity while some could be infectious and even usage of some medications can compromise treatment of patients and will eventually dictate the formulation of the appropriate treatment plan of such patients
These diseases are as follows;
1- Cardiovascular Disorder
2- Endocrine disorder
3- Hemorrhagic disorder
4- Medications
5- Renal disease
6- Liver disease
7- Pulmonary disease
8- Pregnancy
9- Infectious disease
Cardiovascular System
Cardiovascular diseases are the leading global cause of death and is very related to general oral health of a patient.
Some of the most significant cardiovascular disorders in dental treatment include the following;
i- Hypertension
ii- Ischemic heart disease
iii- Valvular heart disease
iv- Arrhythmias
v- Using local anesthesia with vasoconstrictor
i- Hypertension
Arterial hypertension is a common chronic medical conditions distributed among african populations and a significant cause of mortality globally. Estimated high number of people especially adults die of hypertension on annual basis
Systemic hypertension is of two types;
Primary or ‘essential’ hypertension does not have an apparent underlying cause, while secondary hypertension has specific reasons like hyperthyroidism, vascular diseases, and adrenal medullary dysfunction and so on it
Unnoticed or insufficiently controlled systemic hypertension can be a reason for complications in a dental setup. To avoid such, a thorough evaluation of a hypertensive patient visiting a dental clinic is necessary for providing efficient and safe dental care.
Before undertaking any dental procedure, it is advised to do a patient’s complete risk assessment and clinical evaluation with detailed medical and family history as this will assist in assessing the chances of any adverse cardiovascular emergency happening during the dental treatment.
Pulse and blood pressure should measured at every patient visit and in addition, blood pressure monitoring for hypertensive patients with systemic complications must be carried out even during the procedure.
That's all as regards the concise and clear post. More will be discussed on medically compromised patients in the subsequent write-ups. Keep it rocking and rolling. Thanks for the usual support and attention for the posts.
